Please review the Medication Check In proceedures in the MARCHING BAND REGISTRATION GUIDE prior to medication drop off. Appointments for medication drop off will be available for sign up 1-2 weeks before camps and travel. If you need to make a special appointment, send email to medicalsecretary@masonbandboosters.com
DMS FORMS AVAILABLE HERE
Dispensing Medication at School (DMS) Forms & Student Care Plans (SCP)
- All medication (over-the-counter and prescription) must have a DMS form signed by a parent/guardian and healthcare provider.
- Each medication must have its own DMS form.
- DMS forms are valid for up to no more than one full season (unless discontinued earlier by a healthcare provider).

Central OTC Medication
- OTC medications provided centrally by the Band Aid Care Team are: Ibuprofen (Advil, Motrin), Acetaminophen (Tylenol), and Diphenhydramine (Benadryl).

- Students with a DMS form for Central OTC medication can request when needed from a Band Aid.
