Reason for absence/tardy: *Be specific. Give enough information so that directors can determine if the absence should be excused or not. A student may decide with the help of a parent to be absent due to homework, make-up work, visiting relatives, or employment. However, these absences will NOT be "excused".
By submitting this form, we (the student and the parent) understand there is NO guarantee that the absence will be given EXCUSED status. We also understand that it is the student's responsibility to obtain missed assignments and announcements whether the absence is excused or not.
